Prepare For Your First Appointment

We’re pleased that you’ve decided to become a patient. You will really love it here. To help you prepare for your first appointment, we would like to remind you to be sure to bring the following:

•   Proof of Insurance, if applicable
•   Photo Identification (a driver’s license, state-issued ID card, or passport
•   A list of the prescription medications, over-the-counter medications, and supplements you currently take, including dose and frequency
•   Medical/Surgical History
